The coverage cards

# Coverage starts with the exact use and product.

Ask the plan to check the exact medicine, form and reason for treatment. A statement that a drug can be covered is a starting point, not a payment promise.

Ask it in plain words

Will my plan pay for this exact prescription?

## Ask the plan for its own answer

A plan calls its medicine list a formulary. Prior authorisation is the plan's approval step before payment. Both can affect coverage.

- Does my plan's medicine list include this product for the proposed use?

- Is approval needed before the plan pays?

- Who sends the paperwork, and what information is needed?

- What is my cost if the request gets approval? What if it is refused?

## Medicare: the usual plan or the temporary Bridge

As of September 26, 2026, Medicare drug plans can cover Mounjaro (tirzepatide) for type 2 diabetes and Zepbound (tirzepatide) for sleep apnea, where breathing stops during sleep, subject to plan terms.

A body signal named GLP-1 helps control blood sugar and hunger. Drugs that imitate that signal are often called GLP-1 medicines, including tirzepatide.

Medicare's temporary weight-treatment program is called the GLP-1 Bridge. As recorded on September 26, 2026, it includes Foundayo, Wegovy shots and tablets, and Zepbound KwikPen, a pen holding medicine for several shots.

Eligible people pay a $50 monthly copay, meaning their own share of the cost.

July 1, 2026, marks the start of the Bridge; its final day is December 31, 2027. In 2026, one central service handles its approvals and pharmacy payments, separately from your usual Medicare drug plan.

For tirzepatide, only Zepbound KwikPen is included. This is the injection pen that holds medicine for several shots. Vials, small medicine bottles, and pens that hold only one shot are excluded.

- Bridge users need a qualifying Medicare drug plan and must be 18 or older. You also need a prescription and approval before the Bridge pays. The weight and health rules below apply to when this type of treatment began.

- If a Medicare drug plan paid for your GLP-1 medicine prescription in 2026, you cannot access the Bridge that year. Ask your usual drug plan about coverage instead if you have moderate to severe obstructive sleep apnea or type 2 diabetes.

- The Bridge accepts two plan types: a separate Part D plan for medicines, or a Medicare Advantage plan with coordinated care and medicine coverage. Ask the plan which of these types you have.

- Some other Medicare plans, including PACE and private fee-for-service plans, do not qualify on their own. A separate Part D drug plan may allow you to qualify.

- Your weight and height are used to calculate BMI. The Bridge requires a BMI of at least 35, or one of the combinations below. Use the BMI from when this treatment began.

- Certain health problems can qualify a person with a BMI of at least 30: heart failure in which the heart's squeezing action is still normal; kidney disease recorded as stage 3a or higher; or high blood pressure that stays above the program limit despite two blood-pressure medicines. The pressure limit is a top number over 140 or a bottom number over 90.

- With a BMI of at least 27, any one of these can qualify: artery disease with symptoms, a past heart attack, a past stroke, or pre-diabetes. Pre-diabetes means blood sugar is raised before diabetes develops. Artery disease with symptoms is called symptomatic peripheral artery disease.

- The Bridge also requires a continuing food and physical activity plan alongside the prescription.

- A specific liver condition called MASH, a form of fatty liver disease, can also go through the usual drug plan. This applies when there is moderate to advanced scarring without cirrhosis, the severe scarring stage. It does not mean that every fatty liver diagnosis rules out the Bridge.

- Have the person issuing your prescription compare your records with these rules and request Bridge approval if you qualify. Ask your pharmacy whether it can fill a Bridge prescription.

- If the prescription aims to make a heart attack or stroke less likely, ask your usual drug plan about it.

## Savings cards are a separate question

As of September 26, 2026, the tirzepatide maker's savings cards exclude people on Medicare, Medicaid or military coverage.

The savings-card rule does not automatically prevent you from paying the maker's cash price. A cash price is the amount you pay yourself for the medicine, without insurance paying it. Ask which cash offer and terms apply to you.

- Ask what kind of offer the quote uses.

- Ask whether your coverage meets that offer's terms.

- Keep the plan's answer and the service's quote together.
