Your cost worksheet
Turn a quote into a full cost picture.
Use amounts from a written quote. Keep each amount on the same monthly basis. The totals add your entries; they are not prices offered by a care service.
Keep the price terms beside the amounts
A total is useful only when the quote is clear. Leave a price box blank if the cost is unknown. Enter zero only when the service confirms a cost is included or free.
- Confirm the form of the medicine and the quoted supply period.
- Ask whether a fee repeats or only applies at the start.
- Note any discount end date or minimum commitment.
- Keep insurance amounts separate from cash-price offers.
Take the gaps back to the service
Ask for the missing charges in writing, then update the worksheet. Print the page to keep a copy; this tool has no save or send feature.
- Is this charge included in another amount?
- What would change the quoted price?
- What is owed if care does not lead to a prescription?
- How much is due when I leave the plan?
