A card to keep
Take this short list to a provider.
Open this card during a call, or print it and leave room for notes. Ask for each answer in plain words. A blank space is a question to return to.
Print this card with your browser’s print command.
Names to write down
Make the people and the product clear before discussing the rest of the plan.
- Who would assess me? Where is their licence record available?
- What exact medicine and form are you discussing?
- Which pharmacy would fill it?
Care to ask about
Leave the conversation knowing what happens next, including if the answer is no medicine.
- What health history do you need from me?
- What would make this treatment a poor fit for me?
- If I have a concern, how do I contact the person responsible for my care?
- What happens at later visits, or if I want to stop?
Costs to get in writing
Ask for a breakdown you can keep. Put conditions beside the price, rather than relying on the headline.
- What is included in the care fee?
- Is medicine charged separately?
- Which tests, supplies or delivery fees are extra?
- What would I owe if I cancel or no prescription is issued?
An answer worth checking
FDA is short for Food and Drug Administration, the US medicine regulator. Registration puts a business in its records. That entry proves no FDA approval for either the business or its medicines, as recorded on September 26, 2026.
- If approval is mentioned, ask what exact product the claim refers to.
- Keep the licence check separate from any badge or seal.
